579-3Performance of commercial kits available in Brazil for laboratory diagnosis of urogenital mollicute infection

Resumo

Introduction: Laboratory diagnosis of mollicute infections is routinely performed by culture, in the case of Mycoplasma hominis and Ureaplasma urealyticum/parvum in the urogenital tract. M. genitalium, the second most frequent agent of non-gonococcal urethritis, cannot be isolated in the commercially available culture media, and molecular diagnosis is a reality still far from most routine laboratories in Brazil. If the rise in mollicute antibiotic resistance is not enough, we have also observed, from empirical perceptions of routine laboratory staff, the unsatisfactory performance of the most accessible commercial kits in the country for culture of urogenital mollicutes. Material and Methods: The aim of this work was to evaluate the performance of the commercial kits available at the Brazilian market for diagnosis of urogenital mollicute infections, compared to culture media produced at our research laboratory in 99 clinical samples. Results: It was possible to demonstrate that the kit NewProv presented a sensitivity of 17% for Ureaplasma sp. and 33% for M. hominis culture. The kit Laborclin presented a sensitivity of 25% for Ureaplasma sp. culture. It was not observed M. hominis isolation during its evaluation. The kits bioMérieux and International Microbio/Elitech presented a sensitivity of 100%, compared to our in house made media. The kit bioMérrieux also revealed resistance levels of 37.5% for ofloxacin, ciprofloxacin, clarithromycin and erythromycin, and 25% for azithromycin, Discussion and Conclusion: We have observed yet an important level of mollicute resistance to the main antibiotics used in treatment. These results allow a sustained discussion about the status quo of the laboratory diagnosis of these microorganisms in our country also in relation to the cost of the kits with a satisfactory performance and the reimbursement perceived by the clinical laboratory.
